Yoga Class, Nutrition Consultations, and Health Education/Wellness Programmes
I hereby agree to the following:
That I am participating in the yoga classes, nutrition consultation, health and wellness programmes offered by Illuminated Health Ltd. during which I will receive information and instruction about yoga, health, nutrition, and wellness. I recognise that yoga requires physical exertion that may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved.
I understand that it is my responsibility to consult with a physician prior to and regarding my participation in the yoga classes, nutrition, health and wellness programmes. I represent and warrant that I am physically fit and I have no medical condition that would prevent my full participation in the yoga classes, and health and wellness programs.
Yoga is not a substitute for medical attention, examination, diagnosis or treatment. Yoga is not recommended and is not safe under certain medical conditions. I affirm that I alone am responsible to decide whether to practice yoga. If there are any changes to my health, I will notify the teacher prior to the class.
If I experience any pain or discomfort, I will listen to my body, adjust the posture and ask for support from the teacher. If at any point I feel overexertion or fatigue, I will respect my body’s limitations and I will rest before continuing yoga practice.
I acknowledge that participation in yoga classes exposes me to a possible risk of injury. I am fully aware of that risk and I therefore acknowledge that the teacher, any replacement instructor or host facility does not accept any liability for, negligence or other claims arising from or in any way connected with my participation in yoga class, included but not limited to personal property that is lost, stolen or damaged before, during or after yoga class.
I understand and agree that this health, wellness, and nutrition consultation will be based on the principles of Nutrition and Ayurveda, an alternative approach to health, and that this consultation and any information I may receive are different from and are NOT a substitute for modern medical evaluation and treatment or for preventive testing (such as blood tests, colon screening, and any other appropriate screening tests). I further understand that concepts of Holistic and Functional Nutrition may also be included as a part of the recommendations for health. All recommendations are not a substitute for medical treatment.
I understand that a nutrition/wellness/Ayurvedic health consultation, yoga classes, health and wellness classes/workshops/courses/seminars and the recommendations I will receive are not for the purpose of diagnosing or treating any disease that I may have. I further understand that it is not within the scope of this consultation, class or course for the Yoga teacher, Personal Trainer, Ayurvedic Health Specialist, Nutritionist or Course Leader with whom I consult to assume responsibility for the treatment of specific health problems. I understand that what I will be receiving will be advice as to holistic diet and lifestyle choices
I agree to consult with my doctor regarding all matters pertaining to any prescription medication or modern medical treatment that I may be taking. I also understand that this consultation is not a substitute for medical treatment and I will consult with my doctor as needed.
I recognise that no claims or guarantees have been made to me regarding specific medical benefits or improvements in my medical condition(s).
I acknowledge that my personal information will be securely stored by Illuminated Health Ltd for the purposes of running the business and for providing the services that have been requested. Full details can be found on our privacy policy
Nothing in this disclaimer shall limit or exclude any liability that is not permitted under applicable law. Should any provisions of this disclaimer be held to be invalid or unenforceable, the other provisions shall remain valid and enforceable and the disclaimer shall remain in full force and effect. The courts of England shall have exclusive jurisdiction over any dispute or claim arising out of or in connection with this disclaimer.
